Your First Appointment
Please call or come in to set up your first appointment. Among the items we will need on your first visit:
Your current insurance
Please bring your current insurance card and any medical records you want to provide. We can also arrange for transfer of records from other providers if needed.
We encourage you to use our secure online patient portal to update you profile, complete your health history, and e-sign consent forms before your first appointment.
Portal registration can be completed via email invitation from the practice, or by requesting access as a first time user by going to our Patient Portal.
Creating a portal account will allow you to:
- Send secure messages to the practice
- Request appointments
- View and update demographics information
- View and update health history information
- View upcoming appointments
- View lab results
- Make payments
- Search for patient education resources
If you prefer to complete the New Patient Registration Packet on paper, please download, print, and fill out the registration packet PDF and bring the completed forms with you to your first appointment.
Release of Prior Medical Records
Please download, print, and fill out our Medical Record Release Authorization Form so we may obtain your prior medical records before you first visit, if applicable. We can also have the registration form mailed or faxed to you if needed. Click HERE to access the form as a PDF.
We accept all major insurance and Medicare for all medical and primary care services. Please check with our office on rates for medical use of our complementary therapies. We may also be able to help determine whether or not your insurance provides coverage for these treatments.
The Secure Patient Medical Record
When you become a patient, we will walk you through our secure electronic medical record system. You will be able to connect to the program from your home or business computer and your mobile device. With this convenient access, you will be able to check appointments, refill prescriptions, communicate with your providers, review your account, and much more!
