Insurance and Payment Information

We accept all major insurance plans including Medicare, CareFirst, Aetna, Cigna, United Health Care, Johns Hopkins, and Adventist Health for all medical and primary care services. Please check with our office on rates and possible insurance coverage for our complementary therapies. Please bring your insurance card to your first and subsequent visits and let us know if there are any changes in your coverage.

Payment is due for all services on the day of delivery, including co-pays and the full fee for complementary services. Please also be prepared to submit payment for any past balances on your account. You may pay with cash, check, Visa, MasterCard, American Express, and Discover.

Please download, print, and fill out our Medical Record Release Authorization Form so we may obtain your prior medical records before you first visit, if applicable.  Click HERE to access the form as a PDF. We can also mail or fax this form to you if needed.

New Patient Registration – Please Complete Forms

Please download, print, and fill out this  registration packet PDF and bring the completed forms with you to your first appointment. You may also complete your forms by setting up an account in our Secure Patient Portal. See more information below.

We encourage you to use our secure online patient portal from the very start of your relationship with CHI Health Care. On the portal, you can complete your health history and e-sign consent forms even before your first appointment.

You can connect to the portal from your home or business computer and your mobile devices.
